How stem cells are obtained

We base our stem cell treatments on stem cells derived from a patient’s own fat. These fat-derived stem cells are located in the stromal vascular fraction or SVF, which is isolated from fat. SVF is a mixture rich in stem and progenitor cells, including mesenchymal stem cells, endothelial progenitor cells and endothelial cells.

Why use stem cells from fat?

The reasons for extracting stem cells from fat are obvious:

    • Easy availability for autologous use
    • Easily accessible in large quantities with minimally invasive harvesting process
    • No need to expand cells in culture
    • Higher frequency of stem and progenitor cells per removed tissue volume in contrast to bone marrow
    • Extraction of mainly mesenchymal and non haematopoietic stem cells
    • body’s own cells and no donor cells (such as umbilical cord)

MECHANICAL ISOLATION

In this “single stage” process, the body’s own fat is manipulated only minimally so that it retains its actual character.  

The preparation of fatty tissue begins with filtering the fat to remove fibrous fragments. The filtered material is then emulsioned and washed with saline solution WITHOUT the use of enzyme additives or other substances to remove oil and blood residues that can cause inflammation. The end product thus prepared from the body’s own fat tissue thus retains the biological properties of the natural fat-binding tissue and is not subjected to any significant manipulation.

Mechanically obtained stem cells – SVF – have a biomechanical and filling effect when transferred into the intraarticular space and thus fulfil a protective and damping function for the surrounding tissues and organs.

SVF can differentiate into different cell lines such as bone, cartilage and muscle and releases a significantly higher number of growth factors than blood products. Through the additional use of ACP or PRP and the exosomes it contains, cell proliferation and differentiation of obese mesenchymal stem cells located in SVF can be induced.

ENZYMATIC ISOLATION

The removed fat is introduced under absolutely sterile conditions into a sterile closed, fully automated system in the operating theatre – we use the Icellator X for this purpose. Afterwards, the stem cells are separated from the fat aspirate by enzymatic means in a fully automated process. The isolated stem cells are available to the patient after just over an hour.

More or less directly after the fat extraction, the stem cells can be treated with the extracted stem cells.

Stem cell treatment with enzymatically processed stem cells is a minimally invasive outpatient procedure on the same day. The recovery process can and will vary from patient to patient. However, our protocols have been developed and implemented with the goal of minimizing downtime. A general overview of the procedure is as follows:

The first step is water-assisted fat extraction in order to obtain a sufficient amount of fatty tissue.

The unique mixture of stem cells and regenerative cells present in adipose tissue is then isolated and prepared for delivery using a proprietary, closed, sterile, fully automated CGMP cell processing system. In approximately 65 minutes, the highly effective SVF is ready for direct injections and intravenous infusions.

Before the infusion of stem cells and regenerative cells begins, mannitol, a sugar, is administered to help the cells cross the blood-brain barrier for potentially more effective cell delivery.

Adult stem and regeneration cells are administered intravenously according to the individual treatment plan prepared for each patient.

In contrast to mechanically obtained stem cells, the purity and consistency of stem cells obtained by an enzyme means that they can not only be used in large joints or the skin, but can also be introduced into very small joints and above all into the bloodstream. This significantly increases the possibility of therapy, since chronic inflammatory diseases of the CNS such as M Parkinson’s can also be treated, since these cells can be introduced into the bloodstream and thus into the system.

However, the use of an enzyme to isolate cells is currently only permitted in the Bahamas and Japan and is not permitted in either the USA or the EU.
